DEAN Foundation was founded by Mrs. Deepa Muthaiya, who envisioned that DEAN (an acronym for To Dignify and Empower the Ailing and the Needy) would be Read more an oasis for the very sick and suffering who need medical advice, who need to be understood, cared for, listened to and be treated as special. DEAN completed a milestone of 25 years on March 9, 2023; providing medical treatment and supportive care to those with incurable diseases, through Outpatient Care at 3 Centres and Home Care services made available to patients and their families, in the solace and security of their own homes. DEAN Foundation is a registered, non-profit Medical Charitable Trust treating those with serious, life-threatening, chronic and progressive diseases such as Cancer, end-stage diabetes, muscular dystrophy, dementia, multi-organ failure, Alzheimer's, Parkinson's disease, end-stage renal disease, heart diseases, neurological problems, those who are bedridden or house-bound due to stroke, fall, etc., and the frail elderly, with special emphasis on the poor and the marginalized. DEAN Foundation’s Goal: To fulfill a basic human rights issue in reducing the impact of critical, terminal, chronic, life limiting, illness Objectives are to: 1) reduce suffering and improve the quality of life 2) build improved accessibility to medical treatment 3) deliver Community-based and patient-centered care 4) create increased awareness of the availability of specialty service 5) develop, strengthen and enable the efficient, safe and effective running of the community-based Outpatient and Home Care Model that can be replicated Our beneficiaries to whom we provide treatment are the following :  Patients suffering from terminal illness, chronic disease  Bedridden and unable to step out of the house  Unacceptable level of pain or other distress symptoms like vomiting, sleeplessness, fatigue, bleeding ulcers etc. for more than 24 hours  Geriatric related illness Benefits of palliative care service :  Treatment by a Multidisciplinary team at our Outpatient Centres or right at the patient’s doorstep whether a plastic shack, a hut or the pavement - a place they call their "home."  Improves symptom management  Improves quality of life of patients and family  Prolongs survival  Supports the Elderly to deal with advancing illness, frailty and death itself  Reduces overall cost of disease  Causes patients to spend more time at home and reduces the number of hospital inpatients days


Issue

 In India, around 1 million new cancer cases are reported out of which 80% are beyond curative treatment. At present Palliative Care reaches roughly 2% of those requiring end-of-life care.  Palliative Care is not merely the last 48 hours or the last few months of a patient’s life; it is from the time patients are diagnosed with an irreversible life-limiting illness, to their journey’s end. Roughly 40 to 50% of our patients are in the high priority category. As their world and dreams come collapsing around them, patients and caregivers need someone to hear the concerns that overwhelm them. Palliative Care Teams need to get in there early enough to preempt suffering, to prevent harm, to avert patients taking drastic measures like suicide. Patients and caregivers need much handholding as the disease progresses.  Palliative Care though acknowledged is yet to be integrated into the Tamil Nadu State’s Health Care system to the degree that Preventive Medicine and Curative Care have been. Once Palliative Care is given equal importance, every individual would have access to a dignified and pain free end. Till such time DEAN Foundation endeavours to be available to every individual in need.  Hospice & Palliative Care matters. If life begins with an act of love why shouldn’t it end that way? We believe that the end-of-life deserves as much beauty, care and respect as the beginning. Bono of U2 fame sums it up beautifully in this quote, “In life, you try your best to hold on tight to your dignity. In death, sometimes others have to hold on to it for you”. Keeping this aspect in mind, Mrs. Deepa Muthaiya, Founder Chairman, DEAN Foundation said on DEAN’s 25th Anniversary, “Our desire is to achieve a world where all people experience dignity in death as an integral part of their individual life. While we cannot escape death, we want it to be supported by the best palliative care”.


Action

DEAN Foundation has been working from: (i) the Head Office in Kilpauk, since 1998, (ii) the Government Children's Hospital, Egmore and (iii) the upgraded Primary Health Centre in Thiruppukuzhy, Kancheepuram District since 2010 and (iv) the Karamadai Centre, near Mettupalayam, Coimbatore District; Home Care Services since May 2017 We offer our services also at : (i) the Institute of Obstetrics and Gynaecology (IOG) ward attached to the Children's Hospital, Egmore, (ii) the Arignar Anna Memorial Cancer Centre in Karapettai, Kancheepuram and (iii) the Tamil Nadu Government Multi Super Speciality Hospital, Omandurar Besides this, doctor/nurse-led teams provide Home Care Services directly to the homes of poor patients every weekday from Kilpauk, Kancheepuram and Karamadai Centers. We provide Outpatient Care and Inpatient Care to the children at the Children’s Hospital, Egmore, where DEAN has been given 5 beds for Paediatric Palliative Care. For patients, DEAN Foundation offers emotional and spiritual support and medical care, tailored to the patient's wishes to remain at home, as pain free and comfortable as possible. Medical procedures such as IV Fluids, cleaning and dressing of fungating wounds, eye, oral and perineum care, catheterization, cleaning of tracheostomy tube, Ryles Tube insertion, etc are done during every visit as also provision of medicines, medical consumables and non-consumables till the next visit; all of which serve to ease patient’s pain and suffering.

Impact

Since inception in 1998, our teams of doctors and nurses have reached out to 17,866 patients (Adults, Children & the Elderly) as on 30 April 2023 through 77,327Outpatient Services and 64,719 Home Care visits; 13,996 have been Cancer patients. In our Paediatric Palliative Care Project, we have treated 3,540 children between the ages 0 to 12 years of whom 2,212 have been afflicted by Cancer. Currently, approx. 969 patients are under our care. About 85% of our patients are from very low socioeconomic backgrounds; unable to cope with the negative impacts of adverse health events, and 50% are geriatric patients. • Increased awareness of this service created through Volunteers/Opinion Leaders • Comprehensive & quality Palliative Care Services made available to patients at home • Families helped with crucial decisions about traditional healers and stigma • Families save money due to Home visits and by not carrying out unnecessary tests and hospital admissions. • Reduction of anxiety, confusion and helplessness due to patient listening & reassurance of support provided by the team at the doorstep “Living And Dying In Pain: It Doesn't Have To Happen”. DEAN Foundation envisages a community where every individual will have access to Medical Care and a dignified and pain free end.

  • Rural Outreach Hospice & Palliative Care and Geriatric Services, DEAN Foundation Kancheepuram

    On 4 March 2010, DEAN Foundation’s first Rural Palliative Care Centre saw the light of the day, in the Thirupukuzhi Upgraded Primary Health Center Baluchetty Chatram, Kancheepuram District. This in collaboration with the Health and Family Welfare Department, Gov’t of Tamil Nadu, under National Health Mission.
    Home Care visits by a Nurse led team take palliative care to the rural poor and needy of Kancheepuram Block. The expenses for the running of the Project is borne entirely by DEAN Foundation. A doctor from Chennai visits the Kancheepuram Centre periodically for consultations.

    Medical personnel from DEAN Foundation visits the Government Arignar Anna Memorial Cancer Institute, Karapettai twice a week to assist in their Palliative Out-patient Clinic.

On March 28, 2017, SBI Foundation, signed an MoU with DEAN Foundation towards ‘Project SBI - Anugraha’ – a flagship initiative to take Home Care Palliative services to the rural underserved population of Karamadai Block.
