According to WHO, the global maternal mortality ratio (MMR) declined from <\/span><a href=\"https:\/\/www.who.int\/publications\/i\/item\/9789240068759\"><span style=\"font-weight: 300;\">339 to 223 per 100,000 live births <\/span><\/a><span style=\"font-weight: 300;\">between 2000 and 2020, a 34% reduction. Y<\/span><span style=\"font-weight: 300;\">et, the number of deaths is significant, with <\/span><a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/maternal-mortality\"><span style=\"font-weight: 300;\">95% occurring in low- and middle-income countries<\/span><\/a><span style=\"font-weight: 300;\"> (LMICs). I<\/span><span style=\"font-weight: 300;\">ndia\u2019s MMR fell from<\/span><a href=\"https:\/\/www.pib.gov.in\/PressReleasePage.aspx?PRID=2003432\"><span style=\"font-weight: 300;\"> 384 in 2000 to 103 in 2020<\/span><\/a><span style=\"font-weight: 300;\">, contributing to 8% of global maternal deaths, the <\/span><a href=\"https:\/\/www.downtoearth.org.in\/health\/india-recorded-24-000-maternal-deaths-in-2020-second-only-to-nigeria-un-report-shows-87872?utm_source=chatgpt.com\"><span style=\"font-weight: 300;\">second highest globally after Nigeria<\/span><\/a><span style=\"font-weight: 300;\">.<\/span><span style=\"font-weight: 300;\"><br \/><\/span><span style=\"font-weight: 300;\"><br \/><\/span><span style=\"font-weight: 300;\">Child mortality has also improved globally. The under-5 mortality rate dropped from 93 deaths per 1,000 live births in 1990 to 37 in 2022, a 59% reduction. Many of these, including bleeding, infections, preterm birth complications, and diarrheal diseases, can be avoided with timely access to skilled healthcare, improved sanitation, and proper nutrition. However, structural challenges, such as poverty, limited access to antenatal and postnatal care, low immunization rates, and a shortage of healthcare workers\u2014especially in rural India, where 64% of the population lives\u2014intensify these issues. Only <\/span><a href=\"https:\/\/mohfw.gov.in\/sites\/default\/files\/NFHS-5_Phase-II_0.pdf\"><span style=\"font-weight: 300;\">54% of rural mothers<\/span><\/a><span style=\"font-weight: 300;\"> receive the recommended four antenatal visits, and institutional deliveries remain low. Additional barriers, like transportation difficulties and cultural stigmas, further restrict access to essential care.<\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-046fcc4 elementor-widget elementor-widget-image\" data-id=\"046fcc4\" data-element_type=\"widget\" data-widget_type=\"image.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"658\" src=\"https:\/\/give.do\/givegrants\/wp-content\/uploads\/2024\/12\/Group-173-2.png\" class=\"attachment-large size-large wp-image-4455\" alt=\"\" \/>\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-30c807d elementor-widget elementor-widget-heading\" data-id=\"30c807d\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h4 class=\"elementor-heading-title elementor-size-default\">Community Health Workers: A Proven Solution<\/h4>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-03b6048 e-con-full e-flex e-con e-child\" data-id=\"03b6048\" data-element_type=\"container\">\n\t\t\t\t<div class=\"elementor-element elementor-element-657de8b elementor-widget elementor-widget-text-editor\" data-id=\"657de8b\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p><span style=\"font-weight: 300;\">Evidence shows that deploying community health workers (CHWs) is an effective solution. <\/span><span style=\"font-weight: 300;\">A <\/span><a href=\"https:\/\/www.apha.org\/apha-communities\/member-sections\/community-health-workers\/?utm_source=chatgpt.com\"><span style=\"font-weight: 300;\">CHW<\/span><\/a><span style=\"font-weight: 300;\"> is a frontline public health professional who is a trusted member of and\/or has an in-depth understanding of the community they serve. CHWs act as liaisons between health services and the community and play a critical role in implementing simple, cost-effective interventions that address preventable causes, significantly improving survival rates and overall health outcomes. CHWs have existed since the mid-20th century, with early programs like <\/span><a href=\"https:\/\/www.hsph.harvard.edu\/magazine\/magazine_article\/off-the-cuff-barefoot-doctors\/\"><span style=\"font-weight: 300;\">China&#8217;s Barefoot Doctors<\/span><\/a><span style=\"font-weight: 300;\"> in the 1950s. Many global programs, such as India\u2019s <\/span><a href=\"https:\/\/nhm.gov.in\/index1.php?lang=1&amp;level=1&amp;sublinkid=150&amp;lid=226\"><span style=\"font-weight: 300;\">Accredited Social Health Activist (ASHA)<\/span><\/a><span style=\"font-weight: 300;\"> program\u2014one of the largest in the world with 9.8 lakh CHWs\u2014have been successfully implemented since then.<\/span><\/p><p>\u00a0<\/p><p><span style=\"font-weight: 300;\">CHWs perform a variety of roles to support community health, <\/span><a href=\"https:\/\/equityhealthj.biomedcentral.com\/articles\/10.1186\/s12939-023-01944-0#ref-CR3\"><span style=\"font-weight: 300;\">including one or a combination of the following<\/span><\/a><span style=\"font-weight: 300;\">: <\/span><\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-989c25c e-con-full e-flex e-con e-parent\" data-id=\"989c25c\" data-element_type=\"container\">\n\t\t\t\t<div class=\"elementor-element elementor-element-dbb19b4 elementor-widget elementor-widget-image\" data-id=\"dbb19b4\" data-element_type=\"widget\" data-widget_type=\"image.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<img loading=\"lazy\" decoding=\"async\" width=\"1022\" height=\"1024\" src=\"https:\/\/give.do\/givegrants\/wp-content\/uploads\/2024\/12\/Group-176.png\" class=\"attachment-large size-large wp-image-4443\" alt=\"\" \/>\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-d99a68d elementor-widget elementor-widget-heading\" data-id=\"d99a68d\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h4 class=\"elementor-heading-title elementor-size-default\">CHWs have been instrumental in reducing preventable causes of maternal and child mortality across the globe. Mortality<\/h5>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"elementor-element elementor-element-a821eee e-flex e-con-boxed e-con e-parent\" by CHWs increased the proportion of deliveries protected from post-partum hemorrhage (bleeding) from 11.6% to 74.2%, benefiting the poor, illiterate, and those in remote areas.<\/span><\/li><li style=\"font-weight: 300;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Afghanistan<\/span><span style=\"font-weight: 300;\">: A <\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/20053399\/\"><span style=\"font-weight: 300;\">CHW program<\/span><\/a><span style=\"font-weight: 300;\"> distributing misoprostol at home births achieved near-universal uterotonic coverage (92%) compared to 25% in control areas, with fewer adverse effects and 92% of women willing to use it again.<\/span><\/li><li style=\"font-weight: 300;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Burkina Faso<\/span><span style=\"font-weight: 300;\">:<\/span><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/12581438\/\"><span style=\"font-weight: 300;\"> CHWs training<\/span><\/a><span style=\"font-weight: 300;\"> mothers to administer antimalarial drugs reduced malaria progression in children under 6 by 55%, decreasing severe malaria risk from 11% to 5%.<\/span><\/li><li style=\"font-weight: 300;\" aria-level=\"1\"><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/12954560\/\"><span style=\"font-weight: 400;\">Community-based Pneumonia Management<\/span><\/a><span style=\"font-weight: 300;\">: CHWs reduced pneumonia-specific mortality by 42% in neonates, 36% in infants, and 36% in children under 5, while also lowering total of life, more than could be achieved by universal coverage of any other single intervention. A <\/span><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0266613810000549\"><span style=\"font-weight: 300;\">systematic review of RCTs<\/span><\/a><span style=\"font-weight: 300;\"> in LMICs found that community-based interventions significantly improved exclusive breastfeeding rates at 4\u20136 months. Across four studies from different countries, the pooled odds ratio was 5.90, indicating that such interventions substantially increase the likelihood of exclusive breastfeeding in settings where only 25% of infants are exclusively breastfed until 6 months.
