More than one-third of the world's malnourished children live in India. One of the major causes of malnutrition in India is socio-economic inequality. Diets often lack both quality and quantity causing long-term damage to individuals and society. In Shivaji Nagar, more than half of children are stunted, almost every second child is under-weight &amp; the probability of dying by age five per 1000 live births is 51 compared to the national average of 39. The Covid-19 pandemic has further worsened this situation.    As per a report by The Lancet, a reputed medical journal, there would be an estimated 14.3% increase in acute malnutrition among children under the age of 5 years in low-and middle-income countries due to Covid-19 related impact on the country. This would particularly be the case in areas such as Shivaji Nagar, Mumbai, which is adjacent to Asia’s second-largest dumping ground and where the average monthly household income in the community is just Rs. 9,417.  
 Apnalaya influences nutrition practices and knowledge of both mothers and adolescents. We intensively monitor women through pregnancy and children till the age of 6. We ensure the early identification and treatment of nutritional deficiencies in children. We provide malnourished children with appropriate community-based treatment and build referral channels with government facilities. The cost of treating one child with acute malnutrition is just Rs 330.  
 Apnalaya has been working on the issue of malnutrition in Shivaji Nagar since 2010. At present, the project has benefitted 15,848 children and 4,723 women with significant results - the prevalence of underweight children (0-6) has reduced from 59.6% in 2010 to 33% in 2019 and stunting and wasting reduced from 52% and 22% in 2014 to 40% and 12% respectively in 2019. These reductions are accompanied by positive changes in pregnant women’s health and health-seeking behaviour, supporting the assumption that these positive changes will impact the health of their children.   Our overall goal is to improve the nutritional status of stunted and wasted children under 5 years of age, both girls and boys in M East Ward by improving identification, referral and management of undernourished children and enhancing knowledge and awareness of the community and relevant governmental stakeholders.

The city of Mumbai is divided into 24 different parts and the M-East ward ranks 24th (i.e, right at the bottom) with respect to all human development indices. Shivaji Nagar is the biggest cluster of informal settlements in M-East Ward and is situated adjacent to the Deonar dumping ground. Apnalaya conducted a situation analysis of the area and found that there is a lack of government schools above 8th standard. 29% of the children in the age group 6-14 years are out of school in this area, which is quite high a percentage in comparison to the average of13% for the rest of Mumbai. Seven out of ten girls drop out of school before class IX. Dropping out of formal education invariably leads to girls getting married early, which leads to early pregnancies. This results in poor health, and consequently, poor skill acquisition amongst children, and all of it perpetuates a cycle of poverty, debt and poor education. 
 Apnalaya has effectively used ‘peer support’ to support children in pursuing education. We do this by mobilizing school-going girls in small groups and holding them together through a cluster scholarship. A group of fourteen girls and one mentor will form a cluster/unit for this intervention.Model: A cohort of 140 girls studying in grade IX (aged 14-15) will be selected from the 12 clusters of Shivaji Nagar in M-East ward of Mumbai. 14 mentors from the same clusters will be selected to lead this cohort of 140 girls. One mentor will lead a group of 10 girls. These mentors will be young women pursuing higher education, already trained extensively under different projects of our citizenship programme. The mentors will work closely with girls for a period of 4 and half years to ensure the following outcomes: 1.	Continuity in education till grade XII2.	Discouraging marriage while the children are still studying in schools3.	Training the children in a skill to help them have livelihood opportunities4. The mentors will also have regular sessions with the parents, creating a support system for the girls. 
 In our ‘peer support’ programmes in which mentors have worked with girls and their parents, we have found that the positive pressure from the mentors and other girls in the batch helps girls circumvent traditional/patriarchal restrictions.In an area where 38.3% of the girls are married off before the legal age of marriage, less than 1% of girls in our education and citizenship program got married before legal age in 2017-18.A total of 163 girls in the past year were a part of our peer support intervention and were equipped with life skills to negotiate their right to stay in school, to pursue higher education and make informed decisions regarding career and marriage. 10 girls who had dropped out of formal education have re-enrolled into schools.

In urban India, only 24.5% women aged 15 and above participate in the workforce. Research correlates the presence of young children in homes with lower female labour force participation. Apnalaya's recent study (2021) in an urban slum at Shivaji Nagar, Mumbai, suggested unpaid work at home as the primary reason for the female labour force participation to be at a mere 16.8%.  
 Apnalaya’s Community Childcare Centre (CCC) model in urban slums is a social enterprise that supports women in rebuilding and exploring business opportunities, while providing affordable childcare. Apnalaya trains women as entrepreneurs, who provide home-based affordable childcare so more women in their communities are able to seek employment. Each CCC supports six women and four children. At present, 53 CCCs are functioning, providing childcare for 276 children and helping 285 women stay in the workforce.

Impact Ruby Shaikh lives in Chikhalwadi, Govandi. She has 3 children. With her husband being unemployed she was the sole breadwinner of the family. Ruby ran a takeaway lunch catering service for a living. She developed heavy vaginal discharge, abdominal pain and was frequently ill. Societal taboo made her uncomfortable to seek help and she suffered in silence. She was also worried that if she was hospitalized she would lose her sustenance. During a regular home visit, an Apnalaya health worker met Ruby who confided about her problems. She was advised to come to the clinic. The doctor started her treatment. Ruby regularly visited the Apnalaya Gynaecology clinic and completed her treatment course. She is now healthy and has resumed work completely. About The Program Apnalaya runs an initiative to address the issue of maternal, newborn and child healthcare and nutrition in 8 slums in Shivaji Nagar, Govandi in the M/E ward of Mumbai Municipal Corporation. Government schemes and primary health care including maternal and reproductive health care services have been inadequate and ineffective. There are no maternity homes or inpatient health care centers for a population of 6 lakh in Shivaji Nagar. 78% of the population in this area is living in slums and has the lowest life expectancy rate. Apnalaya works towards improving the maternal health care through its antenatal and gynecology clinics. These clinics provide healthcare to women who otherwise have no access to healthcare. When you support this program, you help in the running cost of the expenses of the clinic and help women get access to basic healthcare. About The NGO Apnalaya is a secular voluntary organization, that has been building self-sustaining communities since 1973. It aims to empower the disadvantaged to overcome the social, political and economic barriers and help them lead a better quality of life. They work with the most marginalized people dwelling in the slums of Mumbai. There are 4,200 children under 6 years of age at the center of Apnalaya’s Health Program.The Health and Disability related interventions emphasize on empowering community members to improve health seeking behavior and access healthcare instituted by government agencies. The Livelihood program equips young boys and girls, men and women to identify the potential of opportunities and turn them into economic productivity. They provide sewing classes, enroll and train women who otherwise have been under cultural restraints to stay away from income-generating activities.209 women have economically benefited from Self Help groups organized by Apnalaya. The Education and citizenship program aims to bring together the volunteers from the community so that they have a better understanding of the challenges faced by their community and identify their responsibilities towards it. Help poor women get access to gynecology services

Impact Nafiza married a man whom she loved thinking that she would have a good life ahead. She had no idea how things would turn out. Very soon, **her husband turned abusive. His behaviour did not change even after they had a daughter. He continued to harm Nafiza, and now hurt their daughter too.**He wanted money and did not care how he got it. He convinced Nafiza to be a surrogate mother to childless couples to earn money. He spent all the money on his bad habits and gave nothing to support Nafisa and their daughter. **He forced her to become a surrogate three times.** He abused her even during the pregnancies. When he forced her to do it again for the fourth time, she became frightened. She was in poor health and did not want to get pregnant again. She approached Apnalaya for help. **They explained to her what she had to do to get out of the painful situation she was in. They gave her the courage to speak to her husband and bring him along for counselling.** The center tried to counsel her husband but their efforts were in vain. He continued to abuse her. Apnalaya knew that Nafiza needed the **support of someone she could trust** to keep moving forward. So they encouraged her to tell her brother about the situation.After Apnalaya spoke to Nafiza’s brother, he convinced her to leave her abusive husband, taking her daughter with her. When she did so, her husband became angry and tried to threaten her at first. When Apnalaya warned him with legal action, he left her alone. Nafiza is now staying with her brother’s family. She found a job and is able to provide for herself and her daughter. About The Program Family Counseling Centre (FCC) is a haven for victims of exploitation, violence, infidelity, deprivation and blackmail. The objectives of the program are supporting victims of domestic violence and family conflict through Family Counseling, Support Group (Apnapan) meetings, creating community counselors (Sahara Group), and awareness on women's issues. Apnalaya is a government-appointed service provider under the Domestic Violence Act 2005. Apnalaya trains women to fight domestic violence, conducts awareness sessions and organizes women to come together and help each other. Enable counselling help for domestic violence victims

1 out of 4 children under the age of five have less than their ideal weight in the city of Mumbai (NFHS, 2020). Even in urban areas, malnutrition comes in many forms, from going hungry to eating food without necessary nutrients. It can affect children even before they are born.Nourishment is critical, especially in the early stages of life, for a child as well as pregnant women. If she does not get the right nourishment during pregnancy, it can have a lasting impact on the child. If a breastfeeding mother does not get enough nutrients or is anaemic because of it, the child also lacks what is needed for their growth. This strongly links malnutrition not only to delays in growth but also to different kinds of disabilities.About ApnalayaApnalaya, now in its 50th year, is recognised for its sustained, holistic, and evidence-based work with the urban poor communities of Mumbai. Our urban health care model focuses on nutrition and its impact on the health of mother and child through the use of innovative technological tools.Apnalaya’s intensive engagement ensures that pregnant mothers and children under the age of two have the necessary information and nutrition. The M East Ward is the lowest ranking ward with respect to the Human Development Index (HDI). The area is adjacent to Asia’s oldest and second largest dumping ground and has an average age at death of 39. Residents typically live in 10x15 homes, often without toilets and other sanitation facilities such as piped portable water and waste disposal systems. The average age at death is 39 years, and the average monthly family income is Rs 13,500. According to Apnalaya data, 28% of the population has never attended school; 48% of people above 18 are neither in education, nor in employment and 4.2% families go without food for an entire day. 
 Our programmes include: Health and Disability; Education and Livelihoods; and Citizenship and Advocacy. We work with individuals on issues critical to their survival (such as water, sanitation, nutrition and healthcare), and support their security and development through improved access to education, skills and livelihood opportunities. Through education, we ensure that adolescents continue in formal education, which in turn ensures that they not get married before the legal age. Our health programme helps with safe pregnancy outcomes for expectant mothers. 
 To address these issues, Apnalaya takes a multi-dimensional approach, where we collaborate with the government to ensure provision of basic civic entitlements, help the urban poor communities empower themselves and enable individuals access to basic services, healthcare, education and livelihoods.

Apnalaya works with the urban poor - ENABLING access to basic services, healthcare, education and livelihoods; EMPOWERING them to help themselves; and ENSURING provision of civic entitlements through advocacy with the government.Although urban centres present real opportunities for poor people, they also create and feed conditions within which poverty spreads. Tackling the problem of Urban Poverty is complex. To effect change on the ground, through our three verticals - Health &amp; Disability, Education &amp; Livelihoods and Citizenship &amp; Advocacy - Apnalaya takes a multi-dimensional approach, one where we work with Individuals, Communities and Government at various levels.We work with some of the most underserved communities in Mumbai. Most of our work is focused in the M East ward of Mumbai, an area where people live adjacent to Asia oldest and largest dumping ground, and where, on average 145 people share one toilet seat. The average age at death here is 39, every second child is underweight, women’s participation in the labour force is 9.7% and a family of five typically survives on a combined monthly income of Rs. 13,555.
