Gudalur Valley in Tamil Nadu is a tribal-inhabited valley surrounded by three states: Karnataka, Tamil Nadu, and Kerala.The five tribes living in this area are marginalised and often can’t afford even basic facilities for health and sanitation. The area is hilly and geographically remote, and the nearest hospitals are located particularly far away. 
 We have been running various health programmes for two decades to help tribal communities and create awareness. Now we also run a 20-bed Gudalur Adivasi Hospital to provide free treatment for patients who need intensive care.Every year, an average of 1500 patients are admitted into this hospital from various regions of Gudalur. Non-tribal patients from nearby areas are also provided free treatment at the hospital. 
 This project helps provide timely medical help and free treatment to around 1500 patients belonging to tribal communities in Gudalur. Your donations can help us to save even more people from death through easily treated diseases.

We have seen a lot of funding support has been for equipment, infrastructure and covid related medicine, treatment etc and not for regular medicines being provided. We have been providing this for 2months and plan to continue for another month or two at least.ANC check up and admissions for delivery is utmost important and cannot loose sight of it during this pandemic. 
 Our COVID health care work in the 1st PhaseGudalur Adivasi Hospital is NABH accredited at the entry-level and CMCHIS empaneled hospital. It is one of the two hospitals listed by the Government of Tamil Nadu to take care of Covid-19 patients in Gudalur. A panel of 6 doctors including a surgeon, gynecologist, and anesthetist along with 21 nursing staff are the pillars of GAH. The hospital is a referral care center for the Adivasis who require secondary level care. Apart from the Adivasis, GAH is also popular amongst the non-tribal population of this region for getting good affordable healthcare services. 25% of our beds are allocated for Covid 19 patient admission with all necessary equipment needed.As our Community Health Programme (CHP) has always been the backbone of the work, we were able to take stock and respond quickly and effectively during the 1st wave of Covid 19 last year. Initially, the community was made aware of the symptoms of COVID infection and the precautions to be taken. A list of Health volunteers and contacts were made ready and communicated with all of them to watch for any symptoms and update HAs during the daily evening call. This is further communicated with the doctors and PHCs as needed and further steps are taken to initiate swab collection for Covid19 testing and if positive, to isolate them as per the Government guidelines and instructions.During the lockdown phase, communication with the Village Volunteers, who acted as the point of contact for daily follow-up, was strengthened. Special permission from the Government was obtained to enable vehicles carrying food and medicine to go to the villages. The ambulance was busy all the time, transporting patients needing admission, blood tests, or scans to the hospital, taking the discharged patients home as well as serving as an antenatal checkup center.COVID health care work in the Second phaseHome care for suspected or confirmed Covid 19 In the second wave, we are experiencing more mortality and severity of symptoms, we have restarted calls to health volunteers by HAs on a daily basis and Doctors are updated on the same. If anyone is identified with symptoms or tested positive for Covid19, they will be home quarantined or if an unused house is in the village it is used for isolation. We have listed out village wise isolation plans as certain villages have unused houses or shelter which could be used for this purpose.Health volunteers will be provided basic vitamins and antipyretics at the village level. The next level of medicines will be stocked at the area center so that doctors can instruct the HAs when needed to dispense them at the village level. Heath animators, Health volunteers, and youth will be trained to use pulse oximeters with all precautions. Anyone with a dropping pulse below 94 will be brought to the hospital for further care.To prevent non covid admissions of the vulnerable tribal and reduce the risk of infection spread and complication during this pandemic, Monitoring of people at village level and distribution of medicines ANC admissions and Operational costRationaleImportance of providing regular medications and follow upWhile we focus on Covid 19 through testing, providing medicines, monitoring and hospitalization when needed, we would like to emphasize the need of regular follow up and ensuring the availability of for patients with Chronic illness like Diabetes, Sickle cell disease, Nutritional supplements regular checkup and tests; also their admission for delivery is unavoidable. We have only one private hospital Gudalur. At GAH, 50% of beds remaining 20% for non Covid. Right now given the situation, the non-tribal population and we are the only hospital catering to tribal covid and non covid admissions. Even referral to higher centre 150 kms respectively from Gudalur. At this time availability of beds at higher centres are really difficult.Tribals with co-morbidities are missing the medications due during lockdown, Fear of being affected by Covid and thus just stay within the village.Knowing well that their poor health seeking behavior and added difficulties at present, also that visits to hospital/PHC itself might be exposing the tribals to Covid 19 and the limited number of beds available for Non covid admissions at this point warns us to take necessary steps are taken and ensure regular medicines are provided, regular check up and tests done at the village level.ANC admissions and Operational cost An average of 800 tribal patients seeks outpatient care, 110 inpatient care, undergo 20 surgeries and 25 deliveries every month. Tribal outpatient pay Rs.60 during registration as a contribution toward the medicine expense. Outpatient consultation, investigations and medicines for tribal patients are free. The Medicine and consumable expenses is a large expense and a major deficit in providing the services. The hospital also provides services to non-tribal population with poor financial background and charge a meagre fee for them.Thus there has been a large deficit mainly in the Admissions for delivery and postpartum care. Our main objective is to ensure all pregnant women have access to safe environment for delivery and to ensure that there is no increased maternal morbidity/ mortality which also include referral to tertiary centres when needed. As a result of continued intervention 97% are institutional deliveries in which 79% were at GAH. We seek support in covering the gaps in continuing the maternal care and outpatient services to the tribals,We have around 30 deliveries in a month and costing an average of 4.5lac

Community & Rural Development

The tribal population living in Gudalur are marginalised and often struggle with obtaining basic facilities like health care and education. Literacy rates are so low that agriculture and other related jobs are the most common options for making a living.Training tribal youths in short term vocational skills will be beneficial as their ability to find viable job options in the future will be greatly improved. 
 12 tribal youths from severely disadvantaged backgrounds who have completed their schooling are identified and then trained in paramedical courses at our Ashwini Institute of Health in Gudalur. Most candidates are picked from the school we run for tribal children.These students will choose from courses such as nursing, pharmacy, laboratory technology and other related courses; setting them up for a future career. Considering health care remains a major concern in the region, these same young adults also contribute towards serving the community with their newly acquired skills. 
 This project will identify and train 12 tribal youths in various paramedical courses. Our intent is to help them gain well-meaning employment and positively contribute to the community using their skills. Every donation of Rs. 25,000 sponsors a vocational course for one bright tribal young adult from a marginalised community in Gudalur.

ASHWINI is a registered charitable society working for the health welfare of the Adivasis of the Gudalur and Pandalurtaluks since 1990. Together with sister organizations, ACCORD and VBVT, it works with the Adivasi peoples’ organization, The Adivasi Munnetra Sangam (AMS). 20000 Adivasis, belonging to four different tribes, who are members of the AMS and live in around 320 hamlets spread out over the Gudalur and Pandalur Taluks of the Nilgiris District in Tamil Nadu, are the primary beneficiaries of the project. The majority of them live below the poverty line and literacy rates are still very low. Vision: The primary objective of ASHWINI has been to establish a health system that is “accessible, acceptable, effective and sustainable”. It should be owned and managed by the people themselves, and it should be a system capable of responding to the growing health needs of the Adivasis, and their changing social conditions. Mission: We assure patients quality healthcare through reliable healthcare services, available medicines, and maintainable equipment. We shall ensure the efficiency of operations and effectiveness of treatment through our talented human resources. We shall review this policy for continuing suitability, adequacy, and effectiveness. We shall achieve this through the quality objectives and targets set for various departments ASHWINI has a three-tier health system:• The first tier consists of trained Village Health Volunteers at the village level.• The second tier consists of eight Area Centers (AC) manned by two health Animators(HA) each. They provide preventive and primary curative care.• The third tier is the 50-bedded GudalurAdivasi Hospital. It is equipped with all facilities for secondary-level care The community health program and its outreach services form the foundation of the entire work.Eight area centers have been set up to improve accessibility for the villagers. Each has its own building with minimal infrastructure facilities. Eight area centers are staffed by 16 HAs to provide this gamut of services with the support of a coordinator, doctors, and other professionals. The village health volunteer plays a pivotal role in maintaining the health status of the people. More than 70% of the staff of ASHWINI is from the tribal community, many having been trained on the job. There has been a dramatic improvement in the health indicators over the years, with infant and maternal mortality rates less than the Indian average.

Association For Health Welfare In The Nilgiris

A new generation of students have been trained at the Gudalur Adivasi Hospital under the auspices of the government programme, Bharat Sevak Samaj (BSS). Students have been trained in a variety of critical hospital duties and tasks, including pharmacy management, accounting, basic nursing, and laboratory training. Equipped with these skills and with a formal BSS Diploma, students will be exposed to a variety of employment opportunities including positions at the Gudalur Adivasi Hospital. More importantly, the opportunity to gain a formal education beyond schooling has had a tremendous effect on their families and communities.

Chandrika, an 18-year old girl belonging to the Paniya tribe, is an excellent example of the power of the NRTT-supported education programme. Born to parents who have not had a formal education, she is a first generation student from the Veramanga village.

Chandrika is just a few months away from completing her BSS course in office accounting, and has mastered skills in word processing and basic financial management/organisational accounting. She has received glowing reviews from the senior staff of the hospital in terms of her thoroughness, dedication, and mastery of her work. This is particularly important given that she is likely the first person in her family to have reached this level of education and accomplishment.

Ten-year-old Vasu fell from a tree while he was playing and broke his arm. Since our Gudalur Adivasi Hospital is one of the few hospitals in the region serving the tribal population exclusively, Vasu received immediate care.

Unfortunately, Vasu’s parents marginalised status within a tribal community meant they couldn’t afford their sons treatment.

However, Ashwini and our hospital not only took care of his fracture but also charged an extremely low fee for the whole treatment. Today, Vasu’s arm has healed completely and he can find joy in playing with his friends again.
