For a mother who has spent her entire life loving and raising her child, being beaten and mercilessly thrown out onto the streets by her own son is too heartbreaking to overcome. Basic Healthcare Services (BHS) is a not-for-profit organisation established in 2012, with a focus of high-quality, low-cost primary healthcare services for vulnerable communities. BHS runs AMRIT Clinics in rural, largely tribal communities which provide preventive, promotive and curative services, available 24X7. The 1st AMRIT Clinic was established in early 2013; at present, 6 such clinics cover a population of approximately 90,000 in South Rajasthan. BHS also runs a Primary Health Centre in similar areas, as a public-private-partnership with the state government, which serves a population of 28,000. Together, AMRIT Clinics and the PHC have seen almost 2.75 lakh patient visits of which over half are women, and almost 20% are children. Over 90% of these are tribals, who would have had very few options for seeking care, if these health facilities did not exist. Phulwaris are daycare centres for children under 5; 13 such centres are functional in some of the most remote areas in the region, providing childcare and nutritious food to nearly 400 children.   BHS also conducts research and build strategic partnerships to advance primary healthcare in underserved communities. A notable partnership with IIM-Udaipur, called Primary Healthcare Initiative, is a research and advocacy platform on primary healthcare for the underserved areas. A partnership with Academy of Family Physicians of India enables a Primary Care Fellowship for physicians.

Rural communities in southern Rajasthan comprise of high proportion of tribal populations living in scattered and poorly accessible habitations. The population lives in abject poverty and sees large-scale male migration to cities in Gujarat, Maharashtra and other states. In their absence the entire household responsibility passes on to women and in dealing with the multiple chores- farming, tending to livestock, getting firewood and cooking, childcare is often severely compromised. Young children are left in the care of older siblings, grandparents, or at times even by themselves.  Diets in the region comprise of mainly cereals with very small amounts of pulses, vegetables, oils or foods of animal origin. All these factors contribute to high levels of malnutrition among children- a study we conducted in the region has shown that 33% children under 3 are wasted with 9% being severely wasted; as many as 55% of their mothers are also undernourished (P Mohan, EPW, 2016). 
 BHS runs day-care centres called the phulwaris (meaning a bed of flowers) for young children, which provide a nurturing environment, nutritious food, and engagement and play for cognitive development. Currently, 13 phulwaris operate in very remote and entirely tribal communities in southern Rajasthan providing care and nutrition to nearly 400 children under 5. The value and impact of the phulwaris is very clearly seen with enrolled children showing significant improvement in their nutritional status and clearly seem more alert and playful. The centres have also helped draw attention of the community to childcare and nutrition, and promote linkages with AMRIT Clinics, primary healthcare facilities operating close to the phulwaris. Before the phulwari had started in our village, I would leave my children at home and go to the field. I would be worried about their safety and would keep coming back to check on them. Since the phulwari has started, I am able to leave them comfortably knowing that they are getting good care.  The phulwaris are playing an important role in supporting nutrition of young children in remote, tribal communities in southern Rajasthan. The COVID pandemic and subsequent lockdowns have led to loss of livelihoods, shrinking of wages, and a steep decline in food availability. We are seeing its’ impact in terms of worsening levels of childhood malnutrition. In view of the overall situation of the economy, we are preparing for a prolonged decline in food availability and also continuing high levels of malnutrition.  The value of the phulwaris in the pandemic times is more than ever before. We have now expanded the coverage of the phulwaris to include all children under-5 (covering about 400 children) in their catchment area. We have also resumed the monthly weighing and nutrition education sessions for all children in the neighborhood of the phulwaris, and are now reaching nearly 1000 young children from very poor families. Additionally, the phulwaris have played a role in promoting kitchen gardens and poultry in the hamlets.

Mitigating impact of COVID-19 and lockdown on child nutrition among tribal, high migration communities of South Rajasthan

Basic Healthcare Services(BHS) runs Primary Health Care Services among tribal, high migration communities of South Rajasthan. During the first wave of COVID, BHS provided uninterrupted healthcare, conducted surveillance, addressed fear and myths, and helped contain the epidemic in areas it serves.As the second wave of the epidemic hits the rural areas, we are seeing large numbers of people affected by the disease among the populations we serve, much more than in the first wave. Unprepared health systems and myths and fears associated with the disease are helping spread the disease and are likely to cause high numbers of deaths. District hospitals are already overloaded with COVID patients. There are also fears and mistrust about COVID vaccination.  
 BHS has the longstanding presence of community-based primary healthcare services, technical expertise, and partnerships with public systems. Making use of these strengths, BHS is well-positioned to make meaningful responses for the prevention and management of the COVID pandemic in rural, tribal areas of South Rajasthan.<ol><li>COVID Care Centers: We have helped set up the first COVID care facility in Sabla tehsil of Dungarpur district, equipped with oxygen. We propose to set up two more such facilities in two tribal blocks of Udaipur district, serving a population of about 300,000</li><li>Teleconsultation service: We would set up teleconsultation services for a population of 1,25,000 or more people to be able to connect with Nurses and doctors overcall. The teleconsultation services will allow us to reinforce prevention behaviors, clarify doubts and provide medical advice. </li><li>Assisting home care of infected patients: We would assist families in about 25 panchayats (about 25000 families) to provide home care to the infected individuals through a network of community volunteers</li><li>Promote vaccination among the eligible population: Through the PHC we run as a public-private partnership, we have already vaccinated more than 2000 population. We would address the myths associated with vaccination and ensure complete vaccination of about 8000 additional eligible people.</li><li>Promote awareness on prevention and care for COVID: Many marriages and other ceremonies are leading to the spread of disease. We would help promote individual behavior (masks, ventilation, etc) and also prevent social gatherings.</li><li>Ensure uninterrupted care for women and children: While managing COVID, through our clinics, we would continue to provide care for women and children such as safe childbirth and childhood immunization. </li></ol> 
 We aim to ensure care to about 1000 patients managed at COVID care centers, about 1000 patients supported at households, vaccinate 8000 eligible people, and reach out to a tribal population of 1,25,000 with correct contextual information for preventing and managing COVID.The villages are awash with people fearing the hospitals with the fear of contacting COVID. In such times, a primary healthcare system rooted in the community is critical. As Nanu Bai*, a 55-year old lady, rightly says, “Apni sister tika yahin (sub centre) laga degi”, which translates to “Our Sister will vaccinate us here at the sub-centre (which is closer to home and a short walk away)”. "After working in stone mines in Gogunda for 2 years I went to Gujarat. My room there was small and dark with no light coming in- it had only a small window. There were 10 of us living there. I would leave for work at 8 and come back by about 10 pm. Sundays were also working days, and taking a leave meant getting the salary cut. When I started having fever and cough, I went to the doctor in my lane. I visited him twice, spent about Rs. 5000, but there was no relief. The government hospital was a little far and the doctors there did not check up properly. Over the next 2 months I went to 2 different hospitals, spent around Rs. 20,000, but my disease kept on getting worse. I then came back to my village”- Kalu, 25 year old, resident of Sajnot village in Udaipur.Kalu visited AMRIT Clinic Rawach where he was diagnosed with Tuberculosis. He was started on treatment and provided nutrition support and counselling. The health workers also visited Kalu at home and cleared many doubts that Kalu and his wife had.Stories like Kalu’s can be heard in most villages in southern Rajasthan. The tribal communities here suffer from a high burden of malnutrition and work in occupations which can severely damage their lungs. The government-run health services are unpredictable and of poor quality, and hardly any qualified private health providers are available.Men often acquire tuberculosis at their work sites, spend a lot of money for seeking care, yet in absence of quality health care the disease worsens and they are forced to return home. This places their family members at risk to catch the disease. The absence of a job and income, advanced disease, and incurred debts make the TB treatment weigh heavily on the family. 
 AMRIT Clinics are primary healthcare facilities which provide high-quality, low-cost healthcare in remote tribal communities. A team of physicians, nurses, health workers provides preventive, promotive and curative services which are available round the clock. The team investigates patients for TB and drug resistance and based on the results provides treatment. Patients also receive nutrition support in the form of ingredients for preparing energy- and protein- rich foods at home (oil, gram flour, semolina, roasted Bengal gram and peanuts, and soya nuggets). This support is extremely valuable as almost 70% of TB patients are severely malnourished, yet most have little money and hardly any nutritious foods available at home.Our health worker Ramlal shares, "We see hundreds of patients with TB every month, many of whom come with advanced stages of TB having incurred heavy costs for the treatment. Many patients do not even know that they have TB and the ones who do are afraid to tell others. We counsel our patients about the disease, how to take care of themselves, and what to eat to ensure that they beat the disease"Impact: 
 “Seema, a 15 year old girl was so weak she could not walk and had to be carried to our clinic by her brothers. She was diagnosed with TB. Weimmediately counseled the family and started her treatment. In a few weeks, she started to heal. From walking with the support of a stick, to walking all by herself, at the end of her 6 month course, Seema recovered wonderfully. This was possible because of right quality care + nutrition supplements she received from the clinic”-Ganga, Nurse Coordinator , AMRIT Clinic RawachOver 2000 patients of TB have sought treatment from AMRIT clinics till now. The word of quality care here is spreading far and wide: in 2021, over 600 new patients came to AMRIT Clinics, more than double the number of the previous year. 70% of the patients undergoing treatment recover from TB and also gain an average of 3 kgs in weight. Some patients also become peer educators and peer counsellors who help in linking others suffering from the disease to treatment and support them till they recover.
