The Guru Vishram Vridh Ashram has a dedicated rescue team that saves the elderly from the streets and gives them immediate medical care upon their arrival at the home. To tend to the multiple health complications of the seniors, the home has a medical facility with doctors and nurses, a physiotherapy unit, a pathology lab, a digital X-ray monitoring machine and a pharmacy to provide 24-hour care to older adults anytime they require assistance, even during emergencies. CARE India Medical Society is a 31-year-old public charitable trust dedicated to cancer care in Pune, Maharashtra. The organization provides comprehensive cancer treatment and palliative services, both at home and at its hospital, Vishranti, entirely free of cost to poor patients who cannot afford the expensive treatment costs. The core mission of CARE India is to offer holistic cancer care through a single-window system, making treatment accessible to underprivileged communities while sustaining these efforts through innovative strategies. All services provided by the organization are designed with deep respect for the cultural and social fabric of Indian life, particularly the emphasis on family bonds. In India, families play a crucial role in care-giving, and CARE India’s programs are tailored made to involve families in the patient's treatment and “end of life care “care journey.  In 1994, the organization launched Satseva, India’s first home care program aimed at providing end-of-life care for cancer patients. To further address the need for early detection, especially in women, CARE India introduced Matruseva in 1998, a health maintenance program focused on screening and detecting cancers that are prevalent in women. In 2007, CARE India achieved another milestone by establishing Vishranti, a 22-bed inpatient critical care center. Vishranti provides 24x7 care for home care patients experiencing severe or difficult-to-manage symptoms. The fourth major program, Cantreat, began in 2013 and offers free chemotherapy to women with specific cancers as well as male patients suffering from head and neck cancer. CARE India collaborates extensively with institutions like the Command Hospital and Armed Forces Medical College Pune, ensuring that patients receive free surgeries, investigations, and radiotherapy. These partnerships have enabled the organization to expand its reach and provide life-saving care to a larger number of patients.

Terminally ill cancer patients in advanced stages go through excruciating pain and suffering. Organized pain management and supportive care is scarcely available for advanced stage cancer patients in India. In India, out of the 10 lakh new cases detected every year, 70% are in advanced stages of cancer. Out of these, less than 1% of cancer patients have access to palliative care. The scenario in rural areas is even more grim with non-availability of healthcare facilities for cancer pain management, leave aside any for rural populations. 
 Care India Medical Society is a pubic charitable Trust incorporated in Pune in February 1993. This year we entered into the 29th year of serving the poor and the needy under our flagship program Satseva. Under this program patient-focused medical care is offered to terminally ill cancer patients within the comfort of their homes. The program was India’s first home care program and has been providing free ‘home care’ to patients since 1993.SATSEVA means "Sacred Service" or “Service to God”. In the context of our work, Satseva signifies service to alleviate the pain and human suffering of those battling terminal illnesses such as cancer. Improving the “Quality of Life (QOL)" of cancer patients in advanced stages is the primary objective of Satseva and is achieved by offering Cancer Pain Management as bottom line support.The SATSEVA model of home-care has been successfully implemented in the urban slums of Pune. Over 7,000 patients have been the beneficiaries of this program in the last 28 years. Satseva ROR (Rural Outreach) – Patient Outreach in Rural AreasWith the success of Satseva in Pune, it is our objective to replicate this model in rural parts of Western Maharashtra under Satseva ROR.The goal of the project is to set up Satellite Cancer Pain Management Centers in rural areas. A team of Doctor, nurse and social worker at each Satellite Center will outreach patients living in villages and offer them free pain management services within the comfort of their homes.The first phase of the program involves setting up fully equipped Pain Management Centers in Karad, Satara &amp; Kolhapur. We started the first Satellite center in Karad in April 2021 and hope to open other centers by the end of this year. The budget attached is the cost of setting up and running one center. 
 There is a crying need for Cancer Pain Management in rural areas and we believe that by setting up these centers, we will be able to bring pain relief to thousands of poor and needy cancer patients in rural areas.

Satseva ROR (Rural Outreach)

According to latest WHO statistics, globally there are 18.1 million cases of cancer detected every year and 9.6 million deaths due to cancer every year. 1 out of 6 deaths is due to cancer. This implies that millions of people with life limiting illnesses like cancer live and die in excruciating and untreated pain.The concept of access to pain management as a human right has gained increasing attention in recent years. Commencing as individual advocacy, it was later embraced by the disciplines of pain medicine and palliative care and by mainstream human rights organizations. Today, United Nations and regional human rights bodies have accepted the concept and incorporated it into key human rights reports, reviews, and standards. In India, out of the 10 lakh new cancer cases detected every year, 70% are in advanced stages of cancer. Out of these, less than 1% of cancer patients have access to palliative care. Consequently, there are a large number of cancer patients enter an advanced stage of cancer without treatment and no access to palliative care . Organized Pain Management and Supportive Care is scarcely available for terminally ill cancer patients in India. The end in all such cases is extreme form of human suffering which is unfathomable. 
 The main goal of our project is to provide home-care to poor and needy patients suffering from advanced-stage cancer to improve the quality of their residual life. A team comprising of a doctor, nurse and social worker visit patients in their homes and provides pain and symptom management care for patient relief. Visits are made regularly once the patient’s needs are assessed. Family members are counseled and a holistic approach of providing physical, psychological and spiritual aspects of care is followed. The team ensures that the patients are free from pain and have an improved quality of life. All those patients suffering from difficult symptoms are admitted in Vishranti - a 20-bed 'Critical Cancer Palliative Care Center' which offers 24x7 hospital care to patients. All services such as admission, medicines, nursing and supportive care, investigations and diet in addition to entire medical care are provided totally free of cost. In some specific cases, even the funeral expenses / conveyance of dead body to their villages are provided as humanitarian relief.Inpatient care at Vishranti involves providing professional hospital care free of cost for bringing difficult symptoms under control to mitigate anxiety of members. This effort involves in administering pain killers and proving free medicines, organizing minor procedures/ bypass surgeries to diseased systems to relieve obstruction pain. Once the patients start feeling better, they are sent back to their homes with the reassurance that our home care programs would continue to support them with professional care at home.The scope also involves providing palliative chemo / outsourcing palliative radiation in deserving cases to bring comfort in some cases and improve quality of life for some patients. 
 Our mission is alleviation of pain and human suffering. We wish to reach out to maximum patients in terminal stages of cancer and relieve their pain through our home care and hospital care programs. Our services are primarily for the poor and economically weaker sections of the society who cannot afford expensive treatment and have no access to palliative care.
