For a mother who has spent her entire life loving and raising her child, being beaten and mercilessly thrown out onto the streets by her own son is too heartbreaking to overcome. But this is a common story among the elderly residents of the Guru Vishram Vridh Ashram. “I’ve seen mothers in old and torn clothes, their bodies smelling and their hands and feet rotting and covered in maggots,” shares Dr. G.P. MAHAN Mahatma Gandhi Tribal Hospital & Sant Vinoba Bhave Children Hospital, Jamanalal Bajaj surgical and eye hospital Figure 6 : Hut hospital in beginning Figure: Waiting space for patients Present Hospital (2023) Project Aim: To improve health status of poor tribal of Melghat by reducing deaths and malnutrition and by providing emergency medical care to serious patients, etc. Key activities & Achievement: Treatment of patients by expert physician, eye surgeon and paediatrician. •	Patients treated in the Hospital > 1,27,747 •	Patients admitted and treated in intensive care unit and ward > 8,552 •	Treated > 4,898 serious patients like Heart attack, Brain Haemorrhage, Cerebral Malaria, Meningitis, Tetanus etc. Impact: •	Saved more than 4,458 precious lives in our hospital. (Case fatality rate of <9% which is very less as compared to national average of 18%. Success story:- 1) A, 3 months male child from village Chakarda, wt. 4.350 KG presented with complaints of loose motion, vomiting, difficulty in breathing and breast feeding. On observation, abdomen of the child was distended and the child was lethargic. He was treated by paediatrician as case of severe Pneumonia and? Sepsis with IV antibiotics and supportive treatment for 3 days. After 3 days, child started accepting breastfeed and distention of abdomen was also reduced. The child was discharged after 7 days in good condition. Treatment was free of cost. 2)	 Miracles of MAHAN Trust Melghat! I have heard that life never gives you second change but sometimes it surprises you with 3 to 4 chances, which was proved by “Yohan Baba” ! Farm of Yohan Baba is situated along the river side, which come with an advantage as well as a disadvantage. During the rainy season the river water overflows into the field. Month before Baba was collecting wood for Kitchen purpose from the overflowing river, where suddenly he fainted due to heart attack. Soon the people took Baba to MAHAN Trust Tribal Hospital. At that time Baba was in Heart Failure, the heart rate was merely 35 per minute and the blood oxygen level was only 75 %. Baba wasn't responding when I asked him “Baba Kya ho raha hai ?” Not answering to this he kept his hand on his chest and was trying to say something. Baba had very less time. The Nurse immediately took him for ECG, one nurse recorded BP and there were chances of heart attack in the ECG . Immediately Dr Satav sir started treating Baba with Streptokinase which is an expensive blood thinning lifesaving emergency drug which is not covered under Government supply. Immediately Baba was started on emergency atropine and adrenaline injections and was put on ventilator. We lost hope and thought that now Baba won't survive but Dr Satav acted as a commander of a regiment who controls his commandants at war time which gave confidence to his staff nurse as well as me. Under continuous monitoring of Dr. Satav Sir, Yohan Baba after 18 hours breathed himself and said that the pain was reduced. Heart rates have so far reached to 75 per minute. But due to the water in the lungs, Baba was suffering from aspiration pneumonia, hypoxia and difficulty in breathing. Baba was on ventilator for 4 consecutive days. On the morning of 5th day, Baba himself started breathing without ventilator. Now a days, people even in the city do not get ventilator facilities because of which they lose their lives whereas here in the middle of jungle because of donors ( Bajaj Finance Limited and Caring Friends) we have two ventilators in our hospital which gives life support. My greatest wish for Dr Satav is that he may always achieve success with his faith and determination. Baba improved with his condition but sometimes suddenly his health used to get critical with issues like Low Blood Pressure, reduced oxygen in blood which made me feel that Baba again became critical and we might lose him. But as I stated Life gives many chances which for Baba came in the form of Dr Satav Sir, who saved his life every time. Baba recovered from his heart problem but there were some more secondary infection such as Sepsis (Infection) due to river water in his lungs. In a tribal region such cases are directly referred to metro cities for higher antibiotics, specialist care and ventilator. But here at MAHAN Trust we have both the higher antibiotics, ventilator and specialist care. For the tribal’s of this region this hospital is a dream hospital where we have all emergency facilities available on time which saves the life of people during critical conditions. More and more people are accessing the available facilities as the awareness for healthcare is increasing among the Tribal People. It has also created a need of increasing the Infrastructure of ICU and other required equipment. While leaving from this hospital Yohan baba said “I was lucky enough to have your MAHAN Trust hospital otherwise I would have died in the jungle. I will create awareness about this hospital to such a level that none of our tribal brother will die without treatment “

Healthcare

SAMMAN: 	(Community Based Management for severely malnourished Children: SMC) Project Aim: To reduce the prevalence and deaths due to severe malnutrition (Severe Acute Malnutrition: SAM, Severe Under Weight: SUW) in <5 children in tribal villages of Melghat. Key activities: Trained village health workers (VHWs) treated 1551 SMC (4783 SMC episodes), with MAHAN LTF (Locally prepared therapeutic food) and MAHAN VIT-MIN-MIX (minerals and vitamin supplement) in 33 tribal villages for 90 to 180 days period. Behaviour change communication (BCC) was done for (2,12,275) children and parents episodes(Nutrition demo, flip chart, nail cutting, community growth chart etc.). Impact/Achievement: 1.	At the end of the treatment, 73.6% children came out of severe acute malnutrition. Achieved target of international sphere standards. 2.	SUW recovery rate was 43.90% Better than any project in world. 3.	Recovery rate of all SMC, 44.8%. (With current phase still in progress.) 4.	Only 7 children (case fatality rate-CFR of 0.45% for total children and 0.14% for total SMC episodes) died during treatment. (Very Satisfactory achievement). (Achieved WHO target <4% deaths in SAM children on treatment). 5.	Reduction in prevalence of severe malnutrition in intervention area by 63.79% as compared baseline. From 29.3% (Feb March 2018) to 7.6% (March 2023) .1.	A 23 months old female child was brought to our hospital by our field workers. The child was very critical & suffering from severe malnutrition & severe pneumonia. Her weight was 5.300 kg (expected– 10 kg). We requested relatives to admit the child in the hospital for treatment. However, they refused flatly, saying “she was admitted in various Govt. Hospitals but she could not improve & finally sent back home” & ‘Now it is God’s will’. Lakshmi went home to her remote village in Melghat. After 2 days when MAHAN team visited Lakshmi’s house, they found that Agarbattis (fragrant incense stick) were being burnt in the house and relatives prayed to God. After long persuasion they agreed to accept our treatment. Our village health worker Meerabi (7th std.) treated her at her home for severe malnutrition and pneumonia as per treatment guidelines developed by Dr. Dani, Mrs. Pendharkar and Dr. Satav. Lakshmi showed steep improvement. Her pneumonia was cured. Her weight increased from 5.3 kg to 7.8 kg in 70 days. She is now normal child.

Hunger & Homelessness

Project Aim: To reduce child deaths and malnutrition in <5 children from tribal villages of Melghat. Key activities: Treatment of childhood illnesses like neonatal sepsis, birth asphyxia, Diarrhea, Malaria, Pneumonia, Normal new-born care etc. by VHWs >97,128 illness episodes have been treated so far. Behaviour change communication of 2,16,445 person-episodes. Impact: There is statistically significant reduction in child deaths as compared to baseline (p value< 0.001). U5MR reduced by >75%, low birth weight babies by 48%. SUCCESS STORY 1.	Kokmar is one of the most remote village in dense forest of Melghat. Death is only solution for such critical children as the primary health care is 35 km away from the village with very bad road through dense forest. Divya Khanji Jamunakar, 2 year- 9 Months aged girl, from Kokmar village, had coughing, difficulty in breathing, wheeze and fever (101.3-degree F). Her respiration rate was 48/ min for 3 days (from 9/2/2019). So VHW of the village diagnosed her as case of pneumonia and gave her treatment in the form of Syrup Asthalin, syrup Paracetamol, and Cotrimoxazole. As there was no significant improvement, in 2 days VHW stopped Cotrimoxazole and started dispersible tablet Amoxycillin. After 7 days of treatment, she had no sign and symptoms of pneumonia. She was cured. Hence such treatment by VHW is an angel work in-accessible part of India.

Children

Blindness Control Program Project Aim:  To improve vision of poor tribal and prevent blindness.  Key activities: 1.	Operated more than 2,526 cases with Ophthalmic problems especially cataract (intraocular lens implantation-IOL phacoemulsification, Dacrocystitis, Pterygium, Sac), free of cost. 2.	Door to door Eye check-up of more than 3,21,700 people from > 450 tribal villages. 3.	More than 74,525 students of Melghat were examined. 4.	More than 11,350 students and poor tribal were given spectacles free of cost. 5.	More than 2,603 children were treated with Vitamin A. 6.	Number of spectacles distributed ->4500 Impact:  More than 30,782 patients were given vision and their blindness was prevented.      Success story:-  1)	One poor tribal female from Sawali Village of Madhya Pradesh was blind due to bilateral mature cataract. She wasn’t able to perform her routine activities. Due to poor socio-economic status, she did not have enough money to travel to hospital. During field visit, our team informed her about the free services to poor tribal. She was ready for treatment for his blindness but she was frightened due to certain misbeliefs about cataract surgery. But we counselled her intensively. After this, she became ready for surgery and was brought to hospital by our ambulance. In the first visit she was found to be anaemic with haemoglobin count of 4 gm/dl and after blood transfusion and treatment for anaemia, she was operated for dacrocystitis. In the second visit she was not operated for cataract surgery because of her eye infection.  In the third visit after physician’s fitness and other pre-op tests she was operated for cataract (free). She can see the world now. She can perform her daily activities independently like everyone. She is so happy and so we are.  Work is more important  eye team on wheel: An eye camp was organised in our hospital. Dr. Kavita and her team of dedicated field workers went to Madhya Pradesh for screening cataract patients. While they were crossing the river Tapi, their Jeep was trapped first in mud in river & then on the slope of a hill near the river bank. The river was broad and it was evening and rainy season.  The scene was frightening as there was no village nearby. They could not move the vehicle. Then the team with the help of some travellers, who helped them to put some hard material like stones, dried broken branches of trees below the tyre of jeep. After struggling for more than an hour, they could move the jeep. We realised how difficult life is for the poor tribals, especially serious patients during such rainy season. More than 36 poor tribal patients were operated in that camp.      

MCPEPAG: (Mortality control program for economically productive age group) Project Aim: To reduce death in the age group of 16-60 years from 32 tribal villages. Key activities: Treatment of patients of Hypertension, diarrhoea, Malaria, Pneumonia by VHW(Village Health Workers) in village itself and referral of TB, Coronary Artery Disease, & other diseases for confirmation & management. More than 1,18,808 illness episodes of patients have been treated so far by our VHW’s. Behaviour Change Communication of >1,71,119 person episodes were done. Provided antenatal care to 7078 pregnant women. Provided Local therapeutic food to 3252 pregnant women for 3 to 7 months. Impact: Age specific mortality rate and prevalence of untreated HT has been reduced significantly in intervention area by >50% (p value< 0.001). MMR was reduced by 50%. Success Stories:- 1)	 It’s a story of perseverance and self-realization both on the part of supervisor, village health worker and the patient The patient Bajarangsing Pawar, 40 yrs male from Village Kot, had sustained high blood pressure BP and was being examined regularly for the same. The BP was 154/99 and he was tobacco addict. Village health worker and supervisor did continued counselling for tobacco de-addiction & proper diet. Initially there was total reluctance for change. Despite of the inertness of the patient to change the habits there was a sustained effort by the village health worker & supervisor. The perseverance by the team ultimately made some impact and the ice started melting down. One of the major catalyst / success factors was the village health worker being from the same village community. Patient decided to give up tobacco for 1 week, and that was the turning point of his life. He found his BP coming to normal in the range of 111/81. And eventually he gave up tobacco completely. Till the date whenever there is health assessment in the village, the patient use to come for BP assessment and is quite cautious about the same. He thanks the MAHAN team for showing him the correct path.

Dr. Ashish Satav (MD) and his wife, Dr. Kavita Satav (MS ophthalmology) left their secure jobs at medical college to start MAHAN Trust which provides healthcare in one of the poorest and most deprived parts of India – the Melghat region, where medical facilities were very scarce and under-5 mortality rates were twice that of the national average. MAHAN’s programs include Hospital, Critical Care Facilities, pediatric hospital , Home Based Child Care (HBCC), Malnutrition Eradication (SAMMAN), Providing Locally Prepared Therapeutic Food to malnourished children, Mortality Control Program for Economically Productive Age Group, Blindness Control, De-Addiction Program, Kitchen Garden(KG) and Nutrition Farm (NF) and Counsellor Program for strengthening of government hospitals. Dr Kavita has selflessly supported all of MAHAN’s initiatives and has spearheaded the blindness control program, often personally conducting door to door screenings in over 400 remote villages, spreading awareness to convince tribal to come to the hospital for eye check-ups and surgeries and braving the dangers of the forest environment to personally reach her patients and treat them.MAHAN’s community health programs have reduced child mortality and severe malnutrition by more than 65%, reduced the death rate in economically productive age group and maternal mortality by more than 50%. MAHAN has provided free services of specialists’ doctors to more than 155,000 patients, saved more than 5000 critically ill tribal patients, provided vision to more than 31,000 tribal patients by medical treatment , cataract surgery and providing spectacles and provided free plastic, and other surgeries e.g. cancer, cleft palate, goiter to 4200 tribal. MAHAN could improve 17 government hospitals benefitting &gt;500000 tribal. MAHAN could improve lives of more than 10,00,000 tribal people especially children and women due to their efforts in changing Government policies, expanding 30 state health and ICDS P by research, advocacy and PIL . MAHAN trust research has been published in 34 international medical journals like Lancet and Harvard USA, international conferences as key note speaker e.g. London, has won several national and international awards e.g. World Health Organization’s Public Health Champion Award, felicitation by hon. President of India for social service, advocacy and pioneering medical research. Due to very good work many international organizations e.g. University of Colarado, Denver, USA, Nottingham’s university UK, Respire , UNICEF, etc. have collaborated with MAHAN for its work in Melghat. MAHAN could develop &gt;10600 nutrition farm and nutrition gardens (NF, KG) in 16 tribal villages producing &gt; 300000 kg green vegetables, pulses, etc. by tribal farmers.          Due to MAHAN’s efforts, three villages are free from social drinking of alcohol.

Date of Visit: November 9, 2024 Visited by: Edin Jayanti It takes hours to reach remote tribal areas in the interior of Madhya Pradesh. Quality medical care was a dream for those living in those areas. A dream only fulfilled with the arrival of Mahan Trust. The visit began with a meeting with Dr Ashish, founder of Mahan Trust. We listened to the origins of his foundation; how he was inspired by values of human dignity and compassion for the less privileged and learned of his mission to make healthcare more accessible for those who couldn't afford it. His passion for social service is evident in the various healthcare initiatives undertaken by Mahan Trust. Our visit then delved into the lives of the villagers. MAHAN is providing medical services to the tribal area of Melghat known for very high malnutrition, child mortality and maternal mortality. Most of the villages in Melghat are remote and inaccessible. So, lack of ambulance becomes a major issue while providing health care services and saving lives. Ambulance acts as 'Sanjivani' while working in such conditions. Ours is the only hospital for treatment of critical patients. So; in the absence of emergency medical equipment in our hospital, serious patients require shifting to critical care hospital at least 100 km from our hospital which is very difficult & mostly impossible for poor tribal. 
 MAHAN Trust is providing medical facilities to the tribal population of Melghat. MAHAN’s hospital provides treatment to serious patients, critically ill children, malnourished children; conducts plastic surgery camp, Cataract surgeries, treatment of refractive error, etc. MAHAN also provides community based treatment for childhood illnesses, antenatal care, management of childhood malnutrition, etc. MAHAN provides referral facilities for high risk pregnant women and critically ill patients. MAHAN’s counsellor program is strengthening government hospitals. MAHAN works for community development by mobilising community for utilisation of government schemes, de-addiction and provide a sustainable source of nutrition. MAHAN also works in policy advocacy. 
 MAHAN has built a critical patient and eye hospital, saved more than 1200 serious patients, provided vision to 10000+ poor people and changed lives of 565+ disabled patients by plastic surgeries. Trust was able to decrease the number deaths of children under 5 years of age and severe malnutrition children by more than 64% and 67%; respectively, deaths in age group of 16-60 years by more than 50% and maternal mortality by more than 75% in 14 villages of Melghat. Our successful programs & recommendations have been adopted by the Government for all tribal areas of Maharashtra thus benefiting lacs of tribal. Successful de-addiction of hundreds of people and 3 villages have improved family lives of plenty of people.

Melghat is a hilly, forest, difficult-to-reach tribal area (&gt;4000 km2), having population of 3,00,000 scattered over 320 villages. In Melghat, tribal population is &gt;75%. Tribal are poor people living in huts. Major problems in Melghat are -1)    Very high prevalence of Severe malnutrition: &gt; 20%2)    Very high age specific mortality rate (&gt;80 per 1000 live births in age group of 0-5 years and &gt;400 per one lakh population in age group of 16-60 years) &amp; MMR&gt; 300 .  
 1      Hospital: The needy patients will be treated by Physician, eye surgeon, Paediatrician, obstetricians and surgeons in our hospital. Patient of heart attack, brain haemorrhage, cataract, cancer, post burn contractures, goitres, neonatal sepsis, etc. will be treated in intensive care unit and OT with ventilator, operating microscope, anaesthesia work station, etc. 2    Mortality Control Program for Economically Productive Age Group It consists of detection and management of patients ( age group of 16-60 years) through a trained semiliterate local female village health worker (VHW) from 30-35 tribal villages.3    Home Based Child Care (HBCC):Provision of treatment care to pregnant mothers, newborn and under –5 children in 30 to 35 villages.4    Community based management of severe malnutrition (SAMMAN)It consists of community-based treatment of severely malnourished children along with treatment of associated infections and BCC of parents.5    Behaviour Change Communication (BCC)Intensive BCC e.g. flip-chart, group discussions, audio-visual shows, community meetings, street plays, self-help groups, child to child health education, demonstrations, etc. for nutrition, illnesses, hygiene and sanitation, addiction, safe water, etc.6     Blindness control program:  our team will regularly screen 300 tribal villages of Melghat for ophthalmic problems, conduct BCC, bring needy blind people to our hospital.

The pandemic has impacted the health of children in the rural areas of north eastern Maharashtra. 80% of laborers and pregnant women have been affected. MAHAN Trust is providing therapeutic food to children under the age of 5 to protect them COVID-19, at the cost of ₹7,000. Melghat is difficult to access, hilly, forest, tribal area situated in north east part of Maharashtra. It has scarcity of healthcare infrastructure, population is 300000, tribal constitute 84% of population, most of the people are poor (85%), semiliterate/illiterate tribal (&gt;50%). The doctor patient ratio is &lt;1:10000.Emergency need: Tribal youths (&gt; 30%) temporarily migrated to cities for work. Due to outbreak of COVID 19 in cities and Holi, most of laborers (&gt;20000) have returned back to Melghat from March 2020 and march 2021, who may be COVID carriers.  Due to covid ignorance, un-hygienic living, close community contact, not following covid prevention measures like mask use, physical distance and hand washing and lack of diagnostic facilities, it is disaster of COVID in Melghat due to intracommunity spread. Lot of people are dying due to covid especially young people are also dying.Laborers are affected due to lockdown as they are not habitual of saving money and are dependent on daily wages for food, drugs, etc. Tribal especially, pregnant, lactating mothers and under 5 children (U5C) are most vulnerable for COVID lockdown induced starvation. Due to costly transportation, Melghat tribal are reluctant to go to Amravati for Corona testing.Many patients who are already in serious condition lost their lives due to time consuming difficult travel through mountains and difficult to pay for ICU charges in cities. Without proper diagnostic facilities physicians are afraid of treating patients. Many deaths of non-Corona patients are happening due to corona panic.Melghat is incapable to handle COVID crisis. Due to scarcity of corona testing facility in Melghat, many cases are not diagnosed and many COVID patients are dying because of lack of proper treatment. Undiagnosed asymptomatic/ mild COVID cases are spreading infection in community in Melghat due to ignorance.Fair distribution of resources with special focus on vulnerable area is must.As ICDS has stopped hot cooked food to anganwadis, Under 5 Children malnutrition increased, pregnant and lactating mothers will suffer severe malnutrition and increase maternal mortality rate (MMR), low birth weight (LBW), premature babies and child deaths (U5MR). They are in need of food. There is need of active screening for COVID-19 i.e. nasopharyngeal swab collection, community based COVID research, hospitals for treatment of serious patients, drugs and awareness about COVID.Situation assessment of COVID:As per IIT Delhi more than 3% of tribal are suffering from covid in June 2020. Now it is nearly tripled.After continuous persuasion for 4 months, government has accepted Corona positive cases in Melghat. Government has established sample collection centre and isolation ward/COVID care centre for COVID patients in Dharni. More than 1000 COVID positive cases are present in Melghat. Many youths died of covid in Melghat.  
 A)   Community based interventions: 33 villages. (details about the program proposed for covid tribal village intervention)1.    Intensive behavior changes communication (BCC).Home visits by village health workers (VHWs) and medical and BCC supervisor for corona prevention, travel ban, social distancing, hand washing, self-monitoring for COVID-19 symptoms and consult healthcare for symptoms. We will use one to one counselling, SMS, Digital, social media and audiovisuals.2.    Supply of local therapeutic food(LTF) to all severely malnourished children (SMC). We require LTF 3 to 4 times a day for 300 SMC (3 months).3.    Supply of food to all pregnant and lactating mothers. We will supply balanced diet to &gt;200 pregnant and &gt;200 lactating mothers for 3-6 months. It will reduce maternal mortality rate (MMR), Low birth weight (LBW), premature babies, SMC and under 5 children mortality rate (U5MR).4.    Supply of essential material e.g. face mask, drugs, food, soap, water, etc. to villagers.5.    Supply of pulse oximeter, thermometer, BP apparatus, etc. in villages.6.    Supply of personnel protection equipment’s (PPE) to high-risk people e.g. frontline health care workers and care takers of COVID cases if sufficient funding available.7.     Identification of corona cases- MAHAN will do surveillance of people for COVID-19. Well-trained VHWs can collect Nasopharyngeal swabs (NPS) and do rapid antigen tests in villages. Or will send NPS in Viral transport media to our hospital for further testing e.g. Rapid antigen test/ RTPCR at NIV pune/Amaravati. Lab. Technician/ nurses will collect blood for Antibody testing. Target group-symptomatic cases, dead bodies, relatives/close contacts of confirmed cases and dead persons. Few random asymptomatic people.8.    Immunization by corona vaccine. Our team will help government for immunization of tribal in villages. Especially vaccination of frontline health care workers and high-risk people in community e.g., asymptomatic close contact of confirmed cases.9.    Treatment of confirmed corona cases by drugs by VHWs and medical supervisors.10. Quarantine of indicated persons in villages for 14 days.11.  Real time death tracking.12. Verbal autopsy of all deaths to know cause of death.13. MITS if possible.B)   Hospital based interventions:1)   Identification of corona cases – a)   Diagnosis by rapid antigen test in our hospital.b)   We will collect nasopharyngeal and throat swabs from suspected cases and send to Govt. laboratory, Amaravati, NIV Pune or GMC, Nagpur for RTPCR testing.Or if possible, in our set up;c)    Diagnosis by PCR and antibody test (if available) and development of advanced BSL 3 level laboratories if sufficient funds are given.2)    Treatment of serious COVID cases by drugs, oxygen and ventilators. 3)    Immunization of health care workers by corona vaccine.5)    Isolation wards and ICU development as per international guidelines.6)   Robotic intervention to treat highly contagious corona cases -ICU care when enough funds will be available. 
 a.    Reduction in prevalence of COVID 19 or other respiratory infections in our 30--intervention area (IA) villages.b.    Reduction in deaths and CFR of COVID 19 or other respiratory infections in IA.c.    Maximum number of suspected cases will be screened.d.    Community will become self-sustainable to tackle the issue of COVID 19 or other respiratory infections.e.    Reduction in prevalence of SMC, MMR, U5MR, maternal malnutrition and age specific mortality rate (16-60 years) in 30 intervention villages.f.      Reduction in deaths of serious covid patients admitted in hospital.g.    Improvement of health of covid patients admitted in our hospital.
