But this is a common story among the elderly residents of the Guru Vishram Vridh Ashram. “I’ve seen mothers in old and torn clothes, their bodies smelling and their hands and feet rotting and covered in maggots,” shares Dr. G.P. Bhagat, founder of the ashram. These abandoned elderly men and women lay starving on the streets hoping for someone to rescue them from their suffering. The Guru Vishram Vridh Ashram has a dedicated rescue team that saves the elderly from the streets and gives them immediate medical care upon their arrival at the home. To tend to the multiple health complications of the seniors, the home has a medical facility with doctors and nurses, a physiotherapy unit, a pathology lab, a digital X-ray monitoring machine and a pharmacy to provide 24-hour care to older adults anytime they require assistance, even during emergencies. Human trafficking in India is a horrifying crisis. Thousands of victims, primarily women and girl children, are exploited in red-light areas. Over 10,659 cases were reported between 2018-2022, but experts say that these numbers are likely a fraction of the reality due to underreporting, yet they still do expose the magnitude of the crisis. Girls make up 71.4% of missing children in India and a large number of these girls end up at red-light areas. Sex trafficking is particularly gruesome, with girls as young as 12 sold for as little as a few thousand rupees.

<ul><li>Like in most low or middle-income countries, the need for palliative care is greater in India than in the west simply because disease-specific treatment does not reach patients adequately or early enough. </li><li>Less than 4% of India’s 1.2 billion people have access to Palliative care.</li><li>1 in 5 suicides in India is committed by a person living with an advanced, chronic, or life-limiting condition.</li><li>Due to a lack of basic social security, the financial devastation of the family is far greater at the time of illness.</li><li>While the lower-income groups suffer greatly due to the above reason, paradoxically the affluent in our country are also marginalized, due to the absence of Palliative Care in healthcare institutions, especially in the private sector.</li><li>According to Population Census 2011, there are nearly 104 million elderly persons (aged 60 years or above) in India; 53 million females and 51 million males. A report released by the United Nations Population Fund and HelpAge India suggests that the number of elderly persons is expected to grow to 173 million by 2026.</li><li>The pandemic has brought so much suffering and made us realize the inadequacy of our existing healthcare system. </li></ul> 
 We demonstrate, educate and facilitate the integration of palliative care into mainstream healthcare.<ul><li>Demonstration: providing services in and around Trivandrum, including engaging with local communities. Our flagship facility, Trivandrum Institute of Palliative Sciences (TIPS) is a WHO Collaborating Centre for Training and Policy on Access to Pain Relief. Our interdisciplinary clinical team delivers care to over 4000 patients a year through home care, in-patient, and out-patient services. Other areas of psycho-social support including education, monthly rations, vocation, home modifications for PwD's and elderly, etc. are provided to our beneficiaries to alleviate serious health-related suffering.</li><li>Education: We proactively disseminate learning through in-person and online education. We collaborated with 'Project ECHO India' to widen the horizon to build the capacity of healthcare professionals and spreading awareness among the masses through online education. We have trained more than 4500 healthcare professionals to date across India and South East Asia.</li><li>Facilitation: Advocate for policy and programs and implementation at the community, state, national ( with footprints in 26 states and 6 UTs), and international level. </li></ul> 
 Palliative care is the treatment of serious health-related suffering (SHS). It includes the management of pain and other symptoms and addresses the psychological, social, and spiritual suffering of patients and their families. In a nutshell, this project will remove a significant chunk of serious health-related suffering from our country. This is vitally important considering that less than 4% of the population has access even to basic pain relief in India, that millions die in suffering receiving inappropriate care, that 55 million Indians are pushed below the poverty line by catastrophic health expenditure every year and that more than 26,000 people commit suicide annually because of health-related suffering. To mitigate the suffering, our organisation works with a ‘demonstrate-educate-facilitate’ methodology. At our flagship institute in Trivandrum, we demonstrate quality palliative care reaching people in their homes with the participation of the community. While educating the professionals and the public, we also work with central and state governments for necessary legal and policy changes (like facilitating the 2014 amendment of the NDPS Act by the Indian Parliament, creation of the National Programme for Palliative Care in 2012, the inclusion of palliative care in National Health Policy in 2017 and in undergraduate medical curriculum in 2019). We also work with individuals and institutions to create visible palliative care services, wherever possible integrating them into healthcare. Most importantly, we recognize that the person on the street should be aware and should have a say in his/her healthcare and work with the community to improve its participation in healthcare delivery.We wish to reach the 7 million human beings and families who are going through serious health-related suffering, ensure palliative care is provided to them to improve their overall -Quality of Life
