Society of Community Health Oriented Operational Links, popularly known as SCHOOL is a not-for-profit organization founded in the year 2007. With a strong focus on empowerment, equality and participation, SCHOOL works towards achievement of ‘SDG-3 – Good Health and Wellbeing’ as the last mile, and reaches out to the most vulnerable populations across India. SCHOOL through its flagship initiative ‘Healthy Ageing’ works with vulnerable older people in India.Vriddha Mitra’s vision is ‘an India in which older people can lead dignified, wholesome and secured lives’. It focuses on building an age friendly ecosystem for the elderly population which translates into the ‘change’ in: how we think, feel and act towards the older people; how important it is to develop support structures that foster the abilities of older people; and how we deliver care, support and services that are responsive to the requirements of older people.PROJECT OVERVIEWAgainst each objective which has been developed to address the challenges of vulnerable older people residing in the slums of Pune city, SCHOOL’s ‘Vriddha Mitra’ project undertakes the following activities -Creating social support structure by establishing relationship with the elderlies, their families and community as a whole.11.      Home visits2.      Registering each elderly through saving life check list (SLC)3.      Creating individual care plans based on the needs identified in the ‘Saving life checklist’.4.      Counselling sessions5.      Group meetings Establishing community-based comprehensive service delivery mechanism for basic care of elderlies and activating the public system to provide comprehensive services to the elderlies. 1.      Providing services such as home health check-ups, physiotherapy, virtual OPD,2.      Providing assistive devices; medicines; raw ration; and other basic necessities3.      Referral services for health, violence, legal aid, counselling for mental wellbeing etc. Building linkages with different schemes and programs to enhance inclusion 1.  Linkages with government schemes and NGO programs2.  Support for all documentation required for social security entitlements and schemes  1.      IMPLEMENTATION PROCESS The Community Officers reach out to each elderly through home-visits and community meetings. Every elderly gets registered through an app-based checklist called Saving Life Checklist (SLC). Based on the information collected in the SLC, an Individual Care Plan (ICP) is developed which provides a blueprint for a comprehensive intervention in dealing with the challenges related to physical health, psychological state, level of independence, social relationships, environmental aspects and spiritual concerns of the elderly. All of these together contribute to improvement in the quality of life for the elderlies.DomainsActivities that address these domains1. Physical Health-      Home based health-check-ups, teleconsultations and virtual OPDs-      Disbursement of medicines, multivitamins and supplements-      Referral to higher facilities for treatment2. Psychological state-      One on one Counselling with the elderly-      Following mhGAP tool to understand their mental wellbeing-      Talking and sensitizing the family members3. Level of independence-      Providing assistive devices to the elderly-      Providing basic necessities such as diapers, ration, etc.-      Providing multi-vitamins to improve physical strength to move around4. Social Relationships-      Community-level meetings-      Encouraging youth engagement and Creating volunteers for Inter-generational bonding-      Sensitizing the community regarding the challenges of the elderlies5. Environmental features-      Ensuring there is space, ventilation and access to toilets near the home-      Working towards minor house repairs or constructing houses on the existing space to make their homes livable-      Providing assistive devices to improve mobility and freedom6. Spiritual concerns-      Coming together as a group in group meetings and sharing their beliefs-      Singing, lunching and engaging with each otherTotal reach to date is 10000 elderlies at 4 project locations Pune, Mumbai, Bhopal and Gwalior.
