For a mother who has spent her entire life loving and raising her child, being beaten and mercilessly thrown out onto the streets by her own son is too heartbreaking to overcome. But this is a common story among the elderly residents of the Guru Vishram Vridh Ashram. “I’ve seen mothers in old and torn clothes, their bodies smelling and their hands and feet rotting and covered in maggots,” shares Dr. G.P. Bhagat, founder of the ashram. These abandoned elderly men and women lay starving on the streets hoping for someone to rescue them from their suffering. “During a visit to a nearby village, I found two abandoned babies left to die in a cowshed. Visibly fragile and covered in dirt, the condition of the little ones left me in shock. Moved by the sight, I inquired in the area and was told that these babies were left there by their own family after they were diagnosed with HIV. I immediately brought the babies back home. This was in 2001 and that marked the beginning of a two-decade-long struggle to save abandoned HIV+ children from dying on the streets,” says 70-year-old Mangal Shah, fondly addressed as tai by her 150 adopted grandchildren. Mangaltai founded Palawi with a mission to give abandoned HIV+ children a second chance at life and, more importantly, to right the wrongs that society had done to them. For over two decades, she has rescued hundreds of children off the streets and given them the love they were denied for no fault of theirs. She has given the children life-saving medical care and education while helping them lead normal lives.“It does not cost us money to give them the love they deserve, but the cost of medical care, food, shelter, and education is more than what I can shoulder on my own and I need urgent help,” pleads Mangaltai. He was severely malnourished, sick and had a gaping wound on his head. Seeing the baby’s condition and the maggot-infested wound, doctors gave him six months to live. But Pranay is now 22 years old, he is financially independent and leading a normal life with dignity. What would have happened if I gave up on him?” Mangaltai has faced severe discrimination all along her journey with Palawi, owing to her work and for standing up for the rights of HIV+ kids. She was driven out of schools when she tried to get admissions for her children. “The discrimination against our children was such that they were not even allowed to set foot in any of the schools in this area. We were left with no choice but to start a school of our own,” says Mangaltai.The healing power of loveAccording to Mangaltai, the love that children of Palawi receive is one of the strongest factors in their recovery and healthy childhood. “We are there for these kids throughout the day. These kids have low immunity and are prone to illness. Which is why we need to be extremely cautious. We need to have regular health check-ups to ensure they are safe and healthy. But all of this comes at a cost. Our approaches utilise user-centric and directed treatments, anchored via holistic care frameworks. These include: <ol><li>Emergency Care and Recovery Center (ECRC) and The Shelter: The ECRC in Mogappair has been accessed by over 3,000 homeless people with mental health issues, of whom over three-quarters have satisfactorily transitioned back to their families throughout India. Here people access emergency and critical care services and are provided food, shelter and clothing alongside health, reintegration and aftercare services. The Shelter we run in partnership with The Corporation of Chennai operates similarly for homeless men with mental illness. Post recovery, collective efforts to reintegrate users based on choice and provide needed long term aftercare services to maintain health and wellbeing is offered with the cooperation of carers and families creating pathways for social rehabilitation. </li><li>NALAM - Community Mental Health Care: is delivered through grassroots mobilizers. They identify psychological distress, offering referrals, counselling, clinical care, home visits, home-based care and social entitlements such as citizenship documents, loans, pensions, employment, etc. NALAM clinics spread across urban and rural locations also offer skills development and livelihood initiatives apart from the above mentioned psychological and social interventions for a range of age-groups. In Tamil Nadu, these are offered across 15 service access points in Chennai and Thiruporur districts, reaching out to a population of approximately 7 lakhs. Till date, more than 10,000 individuals have accessed these services with a current active registry of over 2000 individuals. In Thiruporur, Kattumannarkoil and Sriperumbudur Taluks of Tamil Nadu these are administered in partnership with National Health Mission. </li><li>Home Again: offers over 200 individuals with mental illness with sustainable long-stay care needs across a cross-section of low to high disability levels who live in homes as members of families formed in simple rural and urban neighbourhoods with residential daily assistance and case management. Apart from Tamil Nadu where this intervention was first piloted, the model has also been replicated in partnership with the Governments of Kerala and Maharashtra aimed at reducing the incarcerated count at state mental hospitals and the needs of those who continue to remain in psychiatric facilities for over a year or more.</li></ol>

It is the life of most people with persistent mental health conditions to be consigned forever to a care institution. You may wonder why this is such a bad thing – after all, they have food, safety, shelter and treatment. Yes, they do get treated, but not rehabilitated. Yes, they do recover from a disability, but never returned to being fully functional or living a complete life. Social and economic inclusion is but an unattainable dream for the individuals. 
 In response to the longing for home, The Banyan has created an innovative model of long term care. Quite simply we are renting homes in rural and sub urban neighborhoods, encouraging women to form small affinity groups and occupy these homes. Together, they create a shared space of comfort that mimics a familial environment. Every house of three to five women is staffed by a community worker who facilitate the psychosocial and medical intervention they continue to need. The women are supported to engage in a diverse range of work, embrace leisure, recreation and socialization within the community. 
 Over 220 individual live in 49 homes across 3 states and experience an enhanced quality of life due to the opportunities for participation and social mixing offered by the Home Again approach. The approach is being scaled in partnership with State Governments of Kerala and Maharastra, as a means to facilitate exit options into community based living options for long-stay patients in government running mental health centres.The Home Again approach is a high value solution, that improves quality of life, reduces levels of disability, and fosters community integration – while reducing cost of care. The cost of operating a Home Again programme is less than half of what it costs in institutional / hospital based setting.

Creating welcoming spaces for homeless mentally ill

60 Million Indians suffer from mental illness. 130,000 Indians commit suicide every year. 1 in 3 homeless individuals suffer from a mental illness. Clearly, poor mental health is a growing public health concern that affects multiple domains of a person’s life: their relationships with family, friends, colleagues, productivity, physical health outcomes &amp; of course -- their own personal satisfaction and happiness.Many of us remain in our comfortable bubbles of privilege, not realising that 1 in 5 Indians live below the poverty line, and that just as easily could have been us. Adverse experiences with caste and gender based violence, lack of access to education, shelter and food directly affect an individual’s personal and a community’s collective mental health.Common mental disorders are almost twice as frequent in those living in states of poverty &amp; schizophrenia is 8 times more prevalent in those from lower socioeconomic backgrounds.It is no wonder that good health and well being is one of the main sustainable development goals put forth by the UN. Higher levels of stress and isolation, due in part to exponential advancements in technology and unconstrained population growth, has made mental health everyone’s business. It is a problem that needs to be addressed with no further delay.  
 The Banyan has developed a variety of holistic mental health solutions that address multifaceted components of distress that those with mental health issues face. We aim to create replicable &amp; sustainable models of care that address the interconnected elements of mental ill health. This includes, emergency and acute medical, psychiatric, psychological and social care offered through our street based services, hospital settings, shelter based services, community based outpatient care and inclusive long term care options for persons with high support needs.We take service user feedback extremely seriously and all of our programmes and co-created and audited by the people who use them.We approach mental health through the wellbeing and development paradigm, rather than through an illness framework, and seek to ensure an improvement in overall quality of life, for individuals experiencing psychosocial distress.We believe that every individual deserves the right to a dignified and meaningful life, with the opportunity to be aspirational and reach their fullest potential, regardless of the social and structural barriers that have caused them to be left behind and forgotten.

 While we do not have enough resources to ensure only fresh sets of clothing for all our clients, we aim to ensure that they experience agency and dignity in what they wear, and provide each individual in our care with 3 clean and new sets of clothing, including undergarments, each year.  
 Clothing and dressing is just another facet of self-expression. Creating opportunities for each individual to be themselves, and express their individuality through their attire, enhances self-esteem, recovery, and quality of life. Help us offer this experience! Support NEW clothing for homeless persons with mental health issues.

During their time on the streets, many of our clients have gone days without a meal. Others have had to resort to rummaging through garbage and waste, in order to survive. It is no wonder that most of our clients have chronic malnutrition and anaemia at the time of their admission.  
 Meals at The Banyan go well beyond the basics. Not only do we provide freshly cooked meals that are tailored to the diverse cultural roots of the people we serve, but we also mitigate chronic nutritional deficits by providing food that is healthy and nutritious. 
 They are a collective experience through which staff and residents, a like, partake in one of the most basic acts of human connections and bonding. At any given point of time, there are 350 homeless persons with mental health issues residing at our ECRC programmes.
