For a mother who has spent her entire life loving and raising her child, being beaten and mercilessly thrown out onto the streets by her own son is too heartbreaking to overcome. But this is a common story among the elderly residents of the Guru Vishram Vridh Ashram. “I’ve seen mothers in old and torn clothes, their bodies smelling and their hands and feet rotting and covered in maggots,” shares Dr. G.P. Bhagat, founder of the ashram. These abandoned elderly men and women lay starving on the streets hoping for someone to rescue them from their suffering. The Guru Vishram Vridh Ashram has a dedicated rescue team that saves the elderly from the streets and gives them immediate medical care upon their arrival at the home. To tend to the multiple health complications of the seniors, the home has a medical facility with doctors and nurses, a physiotherapy unit, a pathology lab, a digital X-ray monitoring machine and a pharmacy to provide 24-hour care to older adults anytime they require assistance, even during emergencies. As the novel coronavirus (SARS CoV-2) spreads worldwide, lakhs of people succumb to COVID-19 (COrona VIrus Disease), and healthcare systems crumble under the weight, while patients with other ailments mostly slip under the radar of care. The Pandemic across India since March 2020 has made access to scheduled treatment difficult for our child patients, and transportation to get there (in absence of public transport) scarce and expensive. Further, families of many of our patients, being daily wage earners or from out-of-city/state, suffer from a lack of Job, and consequently cannot afford living expenses. Child-patients’ growing bodies need additional nutrients to cope with the medications, and the absence of basic nutrition and hygiene also threatens the safely of the already immunocompromised child. 
 We seek support for the transportation of child patients to and from hospitals for treatment during the COVID-19 pandemic. We seek nutritional support in the form of hygiene kits for some impoverished child patients and their families to support them financially as and when needed.1.   Provision of nutritional support to 10 impoverished child-patients and their families, further disadvantaged during the COVID-19 pandemic @ INR 1000 per child-patient per month.2.   Transportation costs for 6 child-patients from their place of stay to the treating hospital and back @ INR 1,000/- per family per month. INR 1,000/- is calculated for an average distance (up and down) ncluding waiting charges. To minimize risk to the immunocompromised child patients during the epidemic, treatment schedules are being modified such that they only need to visit the treating hospital once a month, unless there is an emergency.   
 Nutrition is the most important need for a child who is on Chemotheraphy.Child-patient growing bodies require nutrition to cope up with the medical effect. They need to build ther immunity to fight the disease and improve their white blood cells count. Most children can be active and feel more healthy when given the best food. They can get back to school and live normal lives like other children. Transportation support helps the child-patient to receive their treatment on time as delay in treatment leads to medical challenges.Golden Butterflies timely support improves the quality of life of our child patients and their familes can lead a life with dignity.

The Lancet estimates that 2.5 million children worldwide die every year requiring palliative care, of whom more than 98% are from developing regions. Children suffering from life-limiting and life-threatening diseases like cancer, irreversible organ failures, HIV/AIDS, neuromuscular or neurodegenerative disorders, etc, have diverse and evolving needs: medical, psychological, financial, social and spiritual. As child-patients navigate the confusing realm of their illness, they need love, laughter, acceptance, and play. Their families, who put up brave faces in front of the child-patients, also suffer and need support.Palliative Care (PC) is defined as an approach that improves quality of life of patients and their families faced with the problems that accompany life-threatening medical conditions. In India, PC is offered in a variety of ways: in-patient, out-patient, day-care, etc. Home-based PC helps patients with chronic and terminal illnesses at their residences. Visits are made by a trained multidisciplinary team consisting of physicians, nurses, social workers, counsellors and volunteers. However, home-based Paediatric Palliative Care is not in vogue in Chennai. Further, research indicates modern society is showing an alarming decline in social connectedness. Building compassionate communities will provide everyone an opportunity to contribute to a healthier and peaceful society.  
 Golden Butterflies (GB) will organize two-hour workshops on the basics of Pediatric Palliative Care and Counselling for volunteers, social workers, students, nurses and doctors with the aim of training a minimum of 3 people in each pin code of Chennai city. Trainings will be held on a monthly basis, with a batch size of minimum 10 people. GB will initiate home-based Paediatric Palliative Care services within Chennai city limits. The integrated multidisciplinary teams will comprise physicians, nurses, social workers, mental health professionals and trained volunteers, who visit child-patients in their homes and offer tailored palliative care plans focused on improving quality of life. Our holistic approach will include provision of medicines, nursing care, counselling, along with practical assistance for the impoverished. We seek support for a home-care vehicle for Golden Butterflies’ team to reach these child-patients in far-flung parts of Chennai city. The vehicle should be medium-sized hatchback able to seat 3 persons along with a driver, and have a spacious boot to carry a medical kit and assistive equipment.  
 These trained volunteers will be called GB WINGS and will work co-ordinatedly with GB in their respective localities to meet the psycho-social, financial, medical, and transportation needs of child-patients in their locality. They will also fulfil the vital role of creating awareness about the need and importance of paediatric palliative care in their local communities. It is generally accepted by the medical fraternity that seriously ill patients often do better when they stay in their own homes and receive personalized treatment. We aim to go to homes of critically-ill child-patients when they need us. Caregiver burnout is a challenge for many families; having access to home-based care provides another layer of direct support along with professional-level advice. Trained volunteers and others on the care team will play a crucial role in coordinating community support options. Trained GB WINGS (volunteers) will be the BRIDGE between HOSPITALS and CHILD-PATIENTS and their FAMILIES, while also creating compassionate communities to enhance the survival of child-patients suffering from life-limiting and life-threatening illnesses.

Furthermore, 7 to 9 million children in the world, ranging from neonates to 18-21 years, could benefit from palliative care. A lot of children with life-threatening and life-limiting conditions require palliative care, including children suffering from cancer, irreversible organ failures of heart, liver and/or kidney, cystic fibrosis, Duchenne muscular dystrophy, HIV/AIDS, neuromuscular or neurodegenerative disorders, severe cerebral palsy, and multiple disabilities such as following brain or spinal cord injury. Palliative care itself is a poorly understood concept in India, and paediatric palliative care an even rarer subset. Children undergo various physiological and psychological changes during their growth, which leads to their unique treatment requirements. 
 Golden Butterflies has Memoranda of Understanding with YRG CARE, Kanchi Kamakoti Child Trust Hospital and Sri Ramachandra Medical College, all in Chennai, for referral of child patients and their families/caregivers, for counselling and providing support for medicines and nutrition. As our beneficiaries hail from low socio-economic circumstances, assistance in cash or kind for food is essential and contributes towards the continuation of ongoing treatment as well as a better quality of life. Further, a few low doses of palliative chemotherapy or palliative radiation are proven to be useful in allowing the patient to live longer and pain-free, in instances such as obstructions due to tumour growth, etc. Golden Butterflies, under this project, intends to comfort its child patients by financially supporting the costs of palliative chemotherapy or palliative radiation and nutrition. 
 1.	10 child patients will be able to undergo palliative chemotherapy or palliative radiation to reduce pain arising from tumour growth. 10 child patients x 50,000 per treatment = 500,0002.	30 child patients will be provided nutrition, including supplements, in cash or kind, for a period of one year. 30 child patients x 15,000 per year = 450,000
